Exchange and cooperation within our network enable us to meet the wishes and needs of our residents and their families at an exceptionally high level of quality.
- Basic care - SGB XI
A key component of home care is basic nursing care. From minor to major personal care, basic nursing care also includes promoting and ensuring mobility and nutrition. We offer a particularly extensive range of services in this area. Some of these tasks can be performed by nursing assistants or nursing aides, as well as by one-year-certified nursing and geriatric nursing assistants.
- Treatment care - SGB V
Another component of home nursing care is treatment care. Special attention is given to patient observation and readiness for intervention. This area of care is staffed exclusively by nurses with at least three years of experience in the fields of health and nursing, pediatric nursing, or geriatric nursing. This area includes all medically prescribed measures such as medication administration, tracheostomy tube care, and all activities related to ventilation and oxygen therapy.

Beatmung
A key component of our services relates to outpatient ventilation. We differentiate between invasive and non-invasive ventilation. We can work with any home ventilator and even train the most common ones independently. In addition, we monitor, control, adapt, and establish ventilation 24 hours a day, according to physician's instructions.
- tracheostomy tube
The tracheostomy tube is one of the most essential components of out-of-hospital intensive care and respiratory care. While it represents an unnatural entry point into the body, with proper care it significantly ensures life support. In addition to tracheostomy tube replacement, endotracheal suctioning, and the selection or conversion of tracheostomy tubes, we are also experts in this field.

Dialysis
The need for a tracheostomy tube can arise for a variety of reasons. We specialize in providing care to residents with a tracheostomy tube and a ventilator. This may require dialysis. We work with outpatient and day-care dialysis centers. We can provide our residents with a qualified nurse during the specified dialysis hours; more often, three times a week for the entire duration of the dialysis session.

Weaning
In addition to caring for our tracheostomy- and/or ventilated residents, it is essential to conduct regular weaning and/or decannulation trials under clinical conditions as part of high-quality care. To this end, we cooperate with a certified weaning center and strictly adhere to our jointly developed weaning and therapy plans. Decannulation means removing the tracheostomy tube. Weaning, on the other hand, means stopping mechanical respiratory support or reducing ventilation times as much as physiologically possible.

Transfer care
There are many different medical conditions, but not every condition necessarily requires permanent care in our intensive care and respiratory care shared units. For example, it's entirely possible for people with chronic lung diseases to be transferred back to their own home after three months, six months, or even a year. This is always our primary goal, and we will support you throughout the process, planning the transition to your own home or to another living arrangement.

Palliativpflege
We aren't always able to discharge our residents back to their own homes. Sometimes illnesses can be more serious than initially expected. We also accompany our residents until the final stages of their lives in a familiar and comfortable environment. We would be happy to advise you on this.

Medical network
Our shared living communities are cared for by local family doctors. The family doctor system provides basic care. These doctors are supplemented by pulmonologists, intensive care physicians, palliative care specialists, pain management specialists, urologists, dentists, etc. Of course, each resident is free to choose their own doctor. We are happy to provide support and advice to ensure the best medical care at all times.

Therapeutic Network
In addition to our medical network, we place particular emphasis on high-quality therapeutic care. We schedule daily therapy sessions Monday through Friday with our speech therapists, physiotherapists, and occupational therapists. On days when no therapy sessions are scheduled, or after therapy has been completed, we incorporate the respective approaches into our nursing care.

Wound management
Adequate wound care is essential in outpatient intensive care and respiratory care. Our decubitus rate among our residents is 0%. We are also happy to treat residents with skin defects and promise the best possible treatment during our 24-hour care. We have not only a nurse with additional qualifications as a wound specialist according to the ICW (International Center for Wound Care) but also a wound therapist for chronic wounds according to the ICW (International Center for Wound Care).

Case management
We plan the transition from the hospital to our shared living communities for our patients. We provide you with comprehensive support throughout the process. We visit all our residents in advance at the hospital or rehabilitation facility, consult with social services and health/nursing insurance providers. We assess the essential support needs and support you in any necessary contact with basic social security offices. We are also happy to arrange admissions to the hospital for routine check-ups.

Housekeeping
Even within shared apartments, strict hygiene guidelines apply, which must be adhered to. Therefore, we pay particular attention to cleanliness. Of course, we cook your meals and do your laundry if necessary. We also clean your apartment several times a day, including the adjoining rooms.

Care services
Whether it's baking or cooking together, playing board games, or reading the newspaper aloud, celebrating parties, attending events, or simply lending a listening ear, we expand your horizons. As far as your loved one's health allows, we try to facilitate all activities of daily living, including going to the movies or visiting family and friends.
